At NorthCare Hospice & Palliative Care, there is a difference. We provide compassionate, state-of-the-art hospice care for people facing a wide range of end-stage illnesses. Our patients are eligible for hospice services when their goal is comfort care rather than curative care. The patient’s doctor and the hospice medical director are asked to certify that there is a life expectancy of six months or less if the illness progresses as expected. NorthCare serves all or part of the following counties in Missouri: Cass, Clay, Clinton, Jackson, Platte and Ray. Since 1995, we have been deeply rooted in our greater Kansas City community, especially the Northland. Our interdisciplinary teams are specialists in hospice care and include medical directors, nurses, hospice techs, social workers, chaplains, therapists and volunteers who work together to create an individual care plan for each patient and their family. The members of our Hospice team are leaders in end-of-life care and bring many years of experience to our patients and their families. Our medical directors are board certified (or in the process of certification) in hospice and palliative medicine. We provide ongoing education for our staff so our patients benefit from the most sophisticated symptom management techniques available today. |
