At NorthCare Hospice & Palliative Care, there is a difference. We provide compassionate, state-of-the-art hospice care for people facing a wide range of end-stage illnesses.

Our patients are eligible for hospice services when their goal is comfort care rather than curative care. The patient’s doctor and the hospice medical director are asked to certify that there is a life expectancy of six months or less if the illness progresses as expected.

A specialized interdisciplinary team of medical directors, nurses, hospice techs, social workers, chaplains, therapists and volunteers works directly with the patient and his/her physician to develop a personalized care plan. This plan addresses the emotional, physical, spiritual, and psychosocial issues that arise for patients and loved ones during their end-of-life journey. Family members are also followed by our bereavement team after the patient dies.
