Who:
Our referrals and requests for information on hospice services come from doctor’s offices, discharge planners, family members and patients themselves. If an individual or family calls to request hospice services, we will contact the patient’s doctor to discuss the request and verify that hospice is medically appropriate.
When:
We recommend that families learn about what hospice care can offer as soon as the doctor indicates that an individual or family member has a life-limiting illness. When you call, one of our intake specialists will speak with you about your specific situation, answer questions and explain how NorthCare Hospice might help. We can also make an informational visit.
How:
Call our main office number at (816) 691-5119. One of our employees will be glad to answer your questions and take referral information. You can also download a Referral Form and fax it to (816) 346-7119.
